Title: Just want to run again - 4/9 with Dr. Su
Post by: Arrojo on March 22, 2012, 10:47:39 AM
I am an avid runner.  Having resurfacing in 18 days from Dr. Su.  I've done 6 marathons, a dozen half marathons and hundreds of shorter races.

In addition, unlike normal runners, I also do sprinting at master's track meets (100 meter dash, 200 meter dash, etc) and have been nationally ranked.

Dr Su says I should be able to sprint again (after full recovery of course), but we'll see.  Just losing one second off my 100 meter dash time means I will not be able to sprint competeitvely again.  But if I can run for fun and excercise that will be good.  Competetive sprinting would be gravy (although I dont think it has ever been done after resurfacing as best I can tell, so perhaps I can help shed some good news on the topic)

I plan on working very hard on rehab and PT (but not too hard to ensure proper healing).  At 6 months, Dr. Su says I can run again, but I will take it verrrry slowly and listen to my body.  I imagine it will be 12 months before I do a hard track workout or plyometrics.  Running is my life.  My mom told me I could run before I could walk.  I am very very active in our running club (was President for 3 years, am currently VP and Track Coach).

As I said, I don't need to be anything special after surgery, I just want to be able to run again and stay fit.  Trying to be realistic and not get my hopes dashed.  I've had a good life.

Most of the post-op running issues I have read about here are soft tissue stuff - scar tissue, etc.  I use Active Release Technique from by guru and plan on having him work on me a lot as I come back to form.  Looking forward to being on the other side.

Had my first scare today - because of my asthma, Dr. Su's office wanted 'my pulmonologist' to certify that I was OK for surgery.  My asthma's never been that bad, but I do need my inhaler before running and hockey.  My pulmonolgist is basically my primary care physician who has known me for awhile.

So I go in and do this pulmonolgy test - blowing as hard as you can into this metered thing while standing up.  The nurse administering the test kept saying that the machine was saying I wasn't taking a deep enough breath.  So, after five times of doing this, I was wheezing.  She said the device couldn't print anything because it was saying my breaths weren't deep enough.  So she leaves the room to get help.  I 'm sitting there thinking, oh great!  I can't believe I failed the test.  Now I can't even get the surgery.  Has this even ever happened?  What am I gonna do if I am not cleared for surgery?

My mind was racing.  She comes back in, and still can't get it top print, and leaves again.  I refused to panic.  But I was still bummed out, until she comes back and finally gets a page of data to print off.  "How's it look?", I asked, already knowing the answer.  She shakes her head.

Fast forward 90 minutes to my meeting with the good Dr.  He seemed quite alarmed at the MoM design of the BHR implant.  I'd tried to helpfully educate him on this - not a THR or recalled implant, Dr. Su is awesome, etc, etc.  Then I asked about the asthma issue and he said, 'oh you're fine for surgery',  The bar's pretty low for not being able to have surgery based on asthma, apparently.  (He did advise me to do 2 puffs/day of my Pulmicort inhaler till surgery.

Alls well that ends well?  Crazy.

It is crazy, but at least you're through that hurdle. We all got the advice against MOM and HR. You're in good hands with Dr. Su.

Arrojo, glad to hear that you have clearance.  I just ordered my first pair of new running shoes in over and year and a half.  I want to protect my new and old joints from old, worn-out shoes.  Amazing to be excited about something like that but getting back into the mindset of "training" with this new hip is like pre-injury days.  So far, the smaller muscles are really good at signalling when they get tired, so its easy to avoid overdoing it, but ...its very, very cool to feel just a little of the runner's joy of old.
Don't know what your running times will be after you recover, but I bet you'll be smiling anyway!

:)  Thanks, Curt.  Yup, I've had plenty of time to make my peace with it.  I will work very hard to get back, but if I can't compete at a high level, so be it.  Just the joy of running will make me smile for years.

I also had a last minute scare.  I went in for the obligatory electro cardiogram.  The report comes back that I had evidence of a previous heart attack.  That is even after I talked to the technician and told him that I have always had a very slow heart rate.

My PCP looked at the report and realized they had made a mistake and knew of my low heart rate due to a lifetime of athletics.  He realized that this would delay my surgery.  Fortunately he had a cardiologist friend that could see me that same day.  I got a second opinion from an expert who took one look at the graph, asked me a couple of questions, and determined I was exceptionally healthy.

@Curt - I got my new running shoes when I started running six weeks ago.  I was also thinking that I am going to give my hip all the help I can.  I even took a running form class even though I have run since I was 13.

Arrojo - I am NOT a runner.....   but, today at 9 1/2 wks post op, I had the urge to run - I felt so free. I know I am not supposed to, so I just kept up my spirited 3 mile walk, but I have to say - for me to even "think" about running was a miracle.  3 more weeks and I think I can get on my Mtn Bike for some easy rides.  Now THAT makes me happy!  Hang in there - you are gonna do great!

Arrojo:

I've spent some time on the site today and sent a few posts that you can check if you want but I want to say a few things about running.

I don't think I saw your age except that you have done some senior sprinting.  I'm 58 now and know that my fastest and longest running days probably are behind me, particularly since I had my right hip resurfacing done by Dr. Pritchett in July of 2011.  But I did do my first 5K run exactly four months after the surgery, and currently run every other day for 35 to 40 minutes and I'm very happy with that just over seven months post-surgery.  I plan to bump that up a bit soon and might do some 10K runs this summer if I feel tip top.  My favorite race always was the half marathon - not sure I'll ever get back to that but its OK if I don't.

As a sprinter, you know about running on your toes and staying level.  What I've discovered is that the best way to run with a resurfaced hip is to sprint very slowly!  I use an exclusive toe/mid foot strike, never ever hit my heels to the ground, have a minimal lift and use a shorter quicker stride - just glide along - my head doesn't go up and down AT ALL.  I'm very happy with this "barefoot" style and actually can build up some decent steam and increase the stride length on a level surface.

BUT - my attitude has also changed completely.  I am blessed by being able to run at all, and treasure just being out on the road.  During the 5K run, I let tons of people pass me by, particularly early in the race, and even caught many of them before the finish - I had nothing to prove (been there, done that).  If my hip starts to act up (it hasn't yet), I won't push it, will cut way back, and start all over again. It sounds like you are on that path too, but be careful.

Thanks, JE. I am 52.  There is no such thing as slow sprinting of course. Trying to jog a 100-meter dash would be humiliating, I think. Nor is there such a thing as barefoot sprinting (where would you put the spikes? :-)  ). So I guess you are saying it cant be done after a hip resurfacing. Perhaps you are right. But Dr. Su told me he thinks it can be done. I guess we'll see. I can only try - the key is to be patient and slowly work my way back.

With 9 days to go, I am working hard, still jogging (slowly and painfully), riding the bike, doing upper body and core. Wouldn't mind losing these extra 5 pounds.

Spent the day yesterday in NY doing pre-op testing etc.  We had to leave the house @ 5:30 AM to catch the train into NYC.  The lack of sleep (havnt been sleeping well due to thinking about this and my job stresses), is giving me a scratchy throat.  I imagine my surgery will be cancelled if I am sick.   :(

You'll be fine, Arrojo - try to relax and let the pros take care of you. After the surgery comes your payoff, and the recuperation work to make it good.

Just got off the phone w/ dr Su office. It's a 'grey area'. My fever is back down, so they (and I) don't want to cancel surgery just yet. We'll just have to see Mon. morning.

Good luck, man. Hopefully it clears up enough that you'll be able to go.

Worst case, you delay, take some more time to prep for the surgery and still come out good in a very short time. Nothing bad in either, just annoyance and dealing with the OA a bit longer. Thinking good thoughts for you.

I had a mild sinus infection about 5 days prior to surgery.  I went to a Doc-in-a-box clinic in a CVS pharmacy, and talked the nice nurse-practitioner into giving me antibiotics.  Cleared it right up.

Hope you get the surgery Arrojo.  I've been running several times per week for a couple months, and even do a little springin/speed work.  Feels great.  Not sure if I'll ever have the wheels like I did a few years ago, but I'm ok with that.

Hello from Arrojos hospital bed! Did indeed have it done today. Dr. Su told me this morning, the worst that could happen due to my past week is that I'd feel lousy. I told him I am already going to feel lousy! Lets just do it!

At 4:00 I was in a lot of pain. But now (10:00) I feel pretty good! I am even doing leg exercises in bed (ankle pumps and heel drags).  I know I know don't overdo it. Walking tomorrow!

Hey congrats Arrojo!!!  Nice to have it turn out well, good call on going forward, stay ahead of the pain and you'll be able to do all the recuperation you can. Don't avoid the meds, if you need them, use them. Nice to have that OA pain gone isn't it?

Thanks hernanu. Dr Su said my hip was in really bad shape but the muscles and bones were very strong. Just had my first PT and walked across the room! I wanted to do more but she was too nervous. She complimented my upper body strength. :)

Day 2: Just walked 200 feet... On crutches! Was only supposed to be using a walker. The PT was amazed. No pain either, just a lot of stiffness. No doubt the painkiller helped. Hopefully they will kick me out tomorrow.

The hospital drugs do help, once they wear off a bit, you'll get some more discomfort and swelling. All of it is normal, so just be ready for it and don't let the pain meds slip if you need them. I was on large amounts of tylenol after being released, but others were on stronger pain medicine.

Whatever works to let you move and start doing your walking and exercise is good. Pleased that you're doing so well.

Had a lousy early morning, was nauseous a couple of times and had a bad headache. They said its likely from the meds. But that passed, I had breakfast and coffee and feel fine now. Saw Sr. Su and he said everything looked great with the incision(dissolvable stitches I can even shower).

Then, I nailed the stairs and took 2 laps around the floor for good measure. The PT loves me. Cleared to leave.

Took the train from NY home yesterday - discharged two days after surgery.   3 hour train ride on the Acela, not too bad at all.  Today showered, shaved, BM (TMI sorry about that), doing exercises, and reading.   Hopefully took my last narcotic overnight and am now just on tylenol. It's in the 50's today ( I despise hot weather) so plan on walking outside a half mile or so later on.

Can't say enough about Dr. Su and HSS, they are amazing.  I also can't emphasize the importance of coming into your surgery in as best shape as you can.  You really need good upper body strength to get yourself in and out of bed as you cannot rely on your upper operated leg at all.   I know everyone has a bad hip (or two) coming into surgery but do whatever you can to maintain fitness and upper body strength is my best advice. All those pushups, situps, pullups, and other exercises I did for months leading up to surgery are paying off and really helping this recovery go smoothly (so far, knock wood).

Happy Birthday to me!  I turned 53 today and am planning on having a beer later.

Happy Birthday!

I got my new hip 6 days after turning 50.  Best birthday present ever.

Take it slow and easy.  After not running for 12 years, waiting 6 months after surgery, and 2.5 months of slow build up I am able to run 27 minutes.  You will be running before you know it.

Best wishes
Dan

Had a little scare last night.  The dog had gotten out so I went to investigate and the gate was open (Daughter apparently didn't latch it tight enough).  While I was out there on the crutches, I accidentally stumbled. I did not fall and instinctively fell toward my good (left leg) to catch my balance.  But I must have twisted a little.

Since then, I've had a little pain in the hip where the implant is.  :(  Not horrible or excruciating pain or anything.  But I am nervous about what I might have done.  I still walked .9 on the crutches this morning on a nice day.

Went through this site scouring for a lot of old posts on cup slippage.  What a wealth of info here - thank you Pat.  Seems like it is highly unlikely for my stumble to have led to anything horrible.  The pain is just a little soreness in the hip and I can still walk.  Actually, my ITB is where I am extremely sore, yikes!

Anyway, about to go walk a mile on crutches before it starts raining!

I think you're fine. We all had setbacks, you just need to get past them and keep going. Just allow for rest, ice, etc. Always keep in mind you're healing first and foremost and will be for a while.

Sounds good - just went to a family reunion over the weekend and spent 7 hours (mostly) dancing to latin music, so the hips got a very good workout.

No excess pain in the hips, but a lot of loose and used up muscles. Cool to do something that would have crushed me pre surgery and no impact on the hips. You'll get there.

Tomorrow will be 3 weeks from surgery. Speaking of threes, it was just three short years ago that I was among the top 50-year-old sprinters in the country. I was invited down to run at the Penn Relays on a 50+  4 x 100 meter dash relay team.

That's me in the red in the center of the photo, running the leadoff leg for Mass Velocity:

I worry about stuff. I see my left (good) leg is still muscular with thick quads, but I appear to have lost muscle mass in the operated leg. Is this normal? It wasn't like that before surgery. It'll be a long road back, but I hope to someday sprint at the Penn Relays again, despite what many runners here seem to think about running fast again.

Was hoping I'd be off crutches by now, but I still need to use one. But after I get walking for a while I can actually walk normally! Walked 1.8 miles today and while I had the crutch with me, I really didn't need it. Just really focused on trying to walk normally.

And, I can actually touch my right foot!

Took me longer to be off crutches totally than I was expecting.  About 3 1/2 weeks.  But if feels good.  My one month checkup is in 2 days.  I am riding the stationary bike 23 minutes, walking about 2/12 miles.  Very happy.  :)

Edited to add: I just tied my shoes!!!

I can't believe it has already been one month (as of yesterday when I had my checkup with Dr. Su).    I have no pain, just a little soreness and stiffness after sitting or lying for long periods.  Dr. Su examined me and said my range of motion was "really good, actually". He gave me a bunch of new stretches to do and also a prescrption for outpatient PT, which I will set up today.  I can add resistance to the bike now.  He also cleared me to drive.  I think my wife is more pleased than anyone about that.

He also cleared me to mow the lawn! I've never been more excited to mow my lawn (not to mention saving the $30 a week I'd been paying the high school kid to do it).

Last night I dreamed I was running.  And running fast!  Then I realized, wait, I'm not supposed to be running yet.  So I stopped.  But it 'felt' good.   :) So glad I had this surgery.  I hope everyone else is too.  Have a great day!

Had my first outpatient PT today.  Just when you think your recovery is going well, a physical therapist tortures you.    :o

It's all good, I know I need to work hard to get back.

This little female PT completely kicked my behind the first time, did it so well I asked for her the second time. She was so excited to be doing her first HR, she researched it to a fare thee well.

Congrats Arrojo!!  awesome progress.  I felt the same way too - thinking I was doing pretty good and then went to PT (physical torture) only to find out that WOW - I have a LOT of work to do.  But it's all good.  It's coming back.

Now you get to be like those of us on the other side and encourage those looking into this for the first time.  First hand testimony is so important.  Can't wait to hear about you running fast!!

Had my first ART (Active Release Technique) session last week and it was terrific.  He broke up all scar tissue that was starting to form in the glutes, perifmormis and TFL.

Spending my days (other than working and coaching track) walking, riding the bike, doing my PT exercises and stretching.  The hardest part is being patient.  I feel great, and so glad Dr. Su did not have the 90 degree restrictions as my ROM is almost all the way back now, thanks to my aggressive 3x day stretching (PT was shocked and pleased).

That's great, Arrojo - I may try that, maybe I could benefit from some scar tissue abuse. The stretching is great, remember that your  muscles are still mending, you want a good balance between good ROM and strengthening.

hernanu, look up activerelease.org and you can find a practitioner near you.  Amazing stuff.

And yeah, only doing the PT excercises every other day for now.

I had my 3 month checkup today.  Dr. Su went through his checklist and said I had 100%.  He tsetd my ROM and said it was terrific. I told him about my accidental running and he thought it was great.  :)

I can ride my bike outside, lift up to 50 lbs, hike up mountains (all of which I intend to do ASAP).   Because I had extra time to kill, I walked all the back to Penn Station (with my heavy backpack of stuff) from his office on the upper east side of NY.  I stopped in Central Park for a snack.

Here is some detailed info for me at 3 1/2 months.  It'll be good for posterity.   :)

Last weekend, I hiked up Wachusett Mountain with a friend.  It was a 3-hour hike.  I've run up that mountain 3 times (a challenging trail/road race), skied it many times, but first time I've hiked it in about 25 years.

Things are going famously.  As noted above, I saw Dr. Su 2 weeks, and he said my Range of Motion (ROM) was all the way back to normal and my strength was great too.  Dr. Su. is very aggressive with stretching and ROM.  He has no 90 degree rule and on the day after the operation he had me bending over in a chair trying to touch the floor.  I did my stretches religiously, and at the one month mark he gave me more advanced stretches.  The hardest was for the periformis.  But by 6 weeks I could put on my own socks and shoes without a problem.

ROM: I have religiously done my stretches 2-3 times a day.  In addition to the periformis, my PT's (who kicked me out after 6 weeks because I had blown through everything they could give me) gave me some good hip flexor stretches, and other stretches.  No doubt all this aggressive stretching has given me the ROM back., but I still do them every day as the periformis is still tight.  I have also had Active Release Technique (ART) done twice to break up all the scar tissue there.

Hip strength: My PTs gave me some great exercises to do.  At one point I was doing them every day, but it was making me very sore, so I cut back to every other day.  In addition to the hip strengthening PT given exercises, they also focused on my core.  So, I do crunches and planks, and a few others core exercises that they gave me.  Around 2 months I started back in doing my situps, pushups and pullups every day.  Dr. Su said I can lift 50 lbs, so I have started doing some easy 50 lb dead lifts as I have free weights in the basement.

Endurance: I try and walk 3 miles every day (the dog loves me).  I was using my bike on a bike trainer for 30 minutes every day, thankfully Dr. Su finally allowed me to ride the bike outside, so I now ride the bike down to the lake and do 30 minutes of aquajogging every other day.  He also said breaststroke (frogkick) is great for recovery so I've been doing that too.

I am eagerly awaiting the 6 month mark when I can start running again.  I will start slowly and take it easy for months 6-12, running only every other day with some easy track workouts.  I plan to start doing plyometrics at the one year mark and I do hope to be competitive again in Masters sprinting next summer, but we'll see.  I told Dr. Su I wanted to get my picture on his wall sprinting (he currently has a skier). :-)

I do need to lose 10-15 lbs as well; that is certainly my goal over the next 3-9 months.

Anyway, I encourage everyone to do your prescribed exercises, and stretches and you will get there.  Hip resurfacing is awesome.

Arrojo, that sounds really good. I was also early on the stretches (always overseen by PT at first), and had no 90 degree restriction. The ROM improvement is great, since you won't have your tight muscles working against you for the more complex activities. The walks sound great (the dogs are probably ecstatic).

One exercise that's been great for me, promoted by my outpatient PT at about 2 months in was to do reverse planks on a pilates ball - where you rest on your back, heels on the top of the ball in front of you then lift up on your shoulders, hold for 5 seconds and down (30 times). Later, at about three months they (me really) upped it to doing the plank and then rolling the ball towards the buttocks while suspended, then back still suspended and down. Very good for the core and stabilizers.

Sounds like you're on your way, take it easy, brother.

I also do the last excercise Hernanu described...it's very hard but it does strengthen the core something fierce! Pulling that ball back and forth is miserable!  Also, roll as much as you can on the foam roller...that hurts too but it helps after you're done!  Glad you're doing great Arrojo!  Dr. Su is aggressive with the therapy but it pays dividends! Good luck with the running...I'm waiting until the 1 year mark before I attempt any, I like the cycling too much anyway!  :)

Hiked up Mt. Wachusett last Saturday with a friend.  A 3-hour hike, not exactly sure how far, but pretty steep especially near the top.  I've run up it 3 times (a very challenging trail race), skied it countless times, but first hike of it in a long, long time.

So, I started running around 5 1/2 months.  Dr. Su said I could run a couple of months ago, but I waited.  Sticking to the track for now.  I actually started on week 8 of the resurfacing to 5K program - 4 sets of 6 minutes running, 1 minute walking.  Except for being out of shape, I felt terrific.  I even ran each interval faster than the previous one.

:)

I love to read this! I'm going to wait a full 6 months, so first run is a week from Friday. I have concerns about the right leg....functioning properly, so I really appreciate hearing about runners with no issue. I can't wait!!!

My six month anniversary is today!  I can now lift > 50 lbs, start basic plyometrics and most important, run on the roads. I celebrated by doing 2 x 12 minutes, with 2 minutes rest (week 11 of the resurfacing to 5K program).  I'm sure I could run longer, but I am sticking to that conservative plan, targeting a Turkey Day 5K.

I was nervous at first, but after a while didnt even think about the hip.  I've missed the roads, but will not forsake trails and the track of course.  I'm not a great distance runner - strictly middle of the pack,  (although I like challenging races), but need to regain my aerobic fitness and plyo srength before I even think about sprinting - hopefully next spring.

Anyway, my return to running, both distance running and sprinting, is still in its infancy, but I am oozing with optimism.  Thank you Dr. Su and my fellow hippies and Pat for this website!

A couple of small milestones today:

1. I started running up stairs two at a time, something I always used to do.  I probably could have done this before today, but I was too nervous to.  It took competetion to do it - dr's office visit (not resurfacing related) was on the 2nd floor and I got to the bottom of the stairs at the same time as a young lady, and wanted to get ahead of her for the appt, so I raced up the stairs.  :)

2. I also ran up my favorite hill for the first time since surgery:

It has now been 8 months (+ 2 days).  I am now running 3-4 miles every other day, and have done one track workout so far.  I hope to do 1-2 track workouts a week.  I have run two 5K races and have started doing P90X workouts.  Many days I have double workouts (run + P90X), although I do not run and do a leg or plyo workout on the same day.

My distance running 5K times (28+) are nowhere near where I feel they should/can be (24-25 range), but I think that is just conditioning, and not any limitation due to the surgery.

I have not yet done any sprinting, still just getting back into distance running shape.  The P90X workouts are terrific for getting my strength back all over, and I feel like things are heading in the right direction.  I don't have a timetable for trying to sprint other then targeting some May/June track meets next year.  But I need to be patient, stay focused and keep doing what I am doing before progressing to the next level of training.

All that said, I will reiterate what I said in my very first post.  If it turns out I can't return to where I was, I have made my peace with it and am delighted to just be back to running.  But I am far from raising the white flag, I will just keep doing what I am doing and see what happens.

Thank you Pat, Vicky, Dr. Su, and my fellow hippies for all you have returned to me.  Happy Holidays to all!

So, I just wanted to catch everyone up on my recovery. i spent the winter doing a lot of skiing, ice hockey, running, and P90 X. I was really happy to be finished with P90X about three weeks ago. Shortly after I finished I started to return to some sprint training, with an eye towards the Connecticut senior games on May 18 where I hoped to run the 50, 100, and 200 m dash.

I strained my back (on the non-operated hip side). Instead of taking it easy and resting it, like someone my age should do, I played hockey through it, did a couple of sprint workouts through it, and even did a plyometrics workout through it. Yup, I'm in idiot, and you guessed it. It got substantially worse.

I'm an optimist by nature, and so I believe that this is not serious and that I just need to rest it and take it easy and then I will be back. I suppose there's still a 20% chance I will run at those senior games in two weeks, but more likely is that I will have to delay my return to competitive sprinting for a little bit. The Rhode Island senior games are in June and I am looking at that. In the meantime I am content to walk the dog and mow the lawn, while   I heal my back With Advil and ice.

Hey Arrojo,
Glad to hear you've been doing well, except for the minor back issue. You know what would be the smart thing to do, but that never stopped us before right?  :)
I know the body core strengthening has helped me much with the running, especially after the 1 year mark going forward.
Best,
David
Off to a trail half-m in the a.m....excited.

Sorry to hear about the strained back, but that's more a normal training screwup.

Sounds good on the overall recovery, you'll get to the RI senior games.

How did the hip hold up with the P90X?

BTW Arrojo 1:54 on my half...which was a hilly trail route with beach run and stuff.
15 minutes faster than last year...
You will notice that it just keeps getting better.
I am finding that running 2x a week short and fast 3-6 miles plus 1 long one on the weekend 10-13 is working well for me.
Happy for you... :)

Thanks for the update.  15 minutes FTLY!  Very cool.  I'll keep that in mind when I start to get into the distance stuff.
