Crossing Midline and Glute “Start up” Pain

I’m a Dr. Gross right hip resurfacing patient who is ~7 months post op.

Previously in my recovery I had significant issues with lying straight leg lifts and “pinchy” groin pain in general. Those issues have since resolved for the most part and I’m back to squatting, deadlifting, and running sprints (albeit not nearly 100% yet).

One lingering issue is crossing my operated leg over the midline of my body when in flexion.I wasn’t allowed to do this for the first 6 months post surgery but since testing it, it’s the one remaining movement that causes “twingy” shock type pain at time. It’s as if the groin seizes up when crossing the midline to protect something. If I leave myself in this position long enough, my body allows for more range of motion, but it is not comfortable.

As a result I notice that when in flexion (lunges, squats, etc.) my operated hip and knee tend to point out away from the body in external rotation. I try to focus on bringing it in but it’s “blocked” in a sense.

Another lingering issue is that when I warm up with body squats and hangs, my glute on the operated side is very slow to “wake up”, and going from flexion to extension of the hips causes some pain in that glute along with some popping type feelings.

Anyone dealt with these issues and if so, how did you approach solving them?

I don't know that I have the exact same experiences that you do since I haven't been doing any weight lifting moves.  I would say that my first resurfacing 11.5 years ago that I stopped doing physical therapy too early.  I had glute and hip flexor pain when running longer distances.

This time, four months after resurfacing the other hip, I am going to physical therapy every other week.  Each time I get new exercises and stretches that cause soreness until the next session.  There is definitely a lot of smaller muscles that need to be exercised.  The big muscles seem to be able to get stronger and mask this weaknesses in the stabilizer muscles.  That is until a certain move that will cause problems.

I highly suggest finding a physical therapist that can give you specific exercises for these other muscles.

I had the same issues with my foot pointing away with squats and feeling blocked when I brought it back in.
Same thing with the flexion and crossing the midline but if I hold it, it gives more.

I put it down to not enough physio too, I find the thigh squeeze onto a rolled up towel is a great one for this as I always did the bands for the opposite exercise but neglected this one.

I'm two years i and its much better but I feel like if I stop doing stretches it reverts back to what it was, possisbly due to about 8 years of arthritis and muscles atrophying or getting used to a limited range of motion.

I don;t get pain if I run but afterwards I feel all the little muscles have been worked thoroughly as thoguh I'm not targetting them properly with other exercise.

I also fell off a ladder as I went down it facing the wrong way in a hurry. Don't do that, fortunately it was just bad bruising but certainly a lesson to be more careful.
