The only thought that I would like to repeat is:  Hip Resurfacing is a more difficult surgery than a THR. Even though a surgeon has had BHR training, he does not become experienced until he does 50 or 100 surgeries.  I know there are many arguments on the Yahoo Surface Hippy Webiste over this statement, but I have studies posted on my website that backs it up.  There is a step learning curve for surgeons learning Hip Resurfacing. Dr. De Smet told me personally that you should choose a doctor that has done at least 100 hip resurfacings because of the level of experienced required. He has done over 2400 and still says he has very difficult cases even with his experience.  I have a whole article written about my thoughts on the subject http://www.surfacehippy.info/pcselectingsurgeon.php  I am about to write a second article about the importance of using a surgeon with experience based on a recent email I recieved from a De Smet hippy that is in Belgium now. She was part of a conversation with Dr. De Smet and here is part of her June 9, 2007 email:


Thom and i had a disturbing conversation with Koen De Smet. He spends 
one or two days a week in town operating, and the rest of his time 
all over the world at seminars and conferences, basically trying to 
convince people that they need to improve surgeon training and 
instrumentation.

He says he is worried about the future of resurfacing. It has become 
so fashionable that loads of OSs have leapt on board and the failure 
rate is now so high that resurfacing is getting a bad reputation. The 
only thing that will help is better instruments and to insist on 
higher standards for surgeon training, which is not happening at the 
moment in the US or, for that matter, in Holland.

Obviously resurfacing is difficult surgery, but so is THR and so are 
lots of other procedures. When you have a difficult procedure that is 
widely needed, instrumentation evolves that makes the surgery easier 
and more accurate.  THR surgery has evolved over 40 years, with 
instrumentation, operating tables, etc. evolving constantly, so it is 
now fairly easy for a competent surgeon to learn the latest technique 
in a weekend refresher course.  But resurf is so new that the 
instrumentation is still in its infancy. As a result, success depends 
heavily on the skill of the surgeon and we all know there are not 
many skilled resurf surgeons around, so the failure rate is high, and 
that's pretty depressing. It also, as Koen points out, gives OSs a 
VERY SOUND reason to say "Go with a THR" if you cannot find a skilled 
resurf surgeon covered by insurance without a one-year waiting list.

I have always explained to people that hip resurfacing, as I have learned about it, is as much an art and a learned skill.  You can learn the skill, but it takes time to earn the experience.  I think the best example of that is sports - a lot of people have the skill to play, for example, football. Even a lot of football players are pros - yet there are still the outstanding professionals that always come thru even in the difficult times. That is what a professional is - they are doing better than most people even when they have a bad day.  You can't buy experience - you earn it.  I think Dr. De Smet's explanation of the lack of good instrumentation for hip resurfacing helps explain why experience and technique is so important. 

I know people will disagree with me - and that is OK. As long as we disagree politely.  I know someone has to be number 1, 5 or 10 with a doctor, but I just don't want to be that person.  I know I only have 2 hips and they need to work properly to walk for the rest of my life.  I did not want to face a lot of revisions if possible.  I know that using a really experienced doctor means my chances of requiring a revision of my BHR will be less.
