Paid cash for surgery / filing insurance claim for facility

I am paying cash to cover the surgeon's fees ( Dr. Gross doesn't accept my insurance), but filing an insurance claim for the facility (Providence NE Hospital). Has anyone else done this? I have TriCare Prime.

While I have not done this, I do work in "insurance".  You may be able to get the forms online, and you also may be able to ask for one of their advocates to help you file it.  Call the customer service line and speak with them first to see who can help you.  Best to try to get it right the first time - as right as you can.  They really do want to help, but sometimes you have to get a few layers up in the chain of command to really get answers, so don't hesitate to ask to speak with someone else.  Good Luck!!
