10 year post op with new and acute groin pain

I had right BHR performed in October of 2008, with much success and little pain for the last 9 years.  Within the last 12 months though, i have developed significant groin pain in the repaired hip while walking/running/playing tennis.  I have been to 2 board certified hip specialists and both have run me through a battery of tests - MRI, XRay, nuclear blood tests, synovial fluid extraction and testing, etc.  Neither Dr can find anything clinically wrong with the hip.  Great news, right!

Except that it still hurts!  So the diagnosis from both independently has been Psoas tendonitis.  So, OK, fine, except that i have no indication of pain when I cycle and the pain is not more accute walking uphill or up stairs (all of which are supposed to be the truest indicators of Psoas issues - at lease that's what i have been told/have read...)

So i am left with still wondering what is going on inside the hip.  Any help from anyone would be greatly appreciated!

I had the same problem. Took 5 months of psoas stretching before I noticed some improvement. I still stretch both sides once a day and 13 months on, I've just regained our club Racketball cup. In 2017, I struggled to walk 3 miles. The stretch, left knee at 90 degrees, right leg straight, pelvis forward with right arm raised straight and leaning over to left. 20 seconds or as long as you can hold it. Then other side for balancing the body and then the first stretch again. I play squash once a week but the tendon requires more stretching before I can return to team squash. Hope this helps.

Prp can help psoas tendinitis. So does glycerol trinitrate patches.  That’s if eccentric exercises don’t help. Make sure you back off and don’t aggravate it whilst doing rehab.

I have it pretty bad and am still struggling but getting there slowly.

When you say groin pain, can you be more specific regarding pain location?  My concept of groin pain is on the inside of the upper thigh, all the way immediately adjacent to the pelvis, right at the connection for the adductor longus and/or the gracilis muscles.  I associate psoas pain as toward the front of the upper leg right where it connects to the hip just along and inside side the flexors and slightly higher toward the abdomen.  I would hope that good stretching done cautiously and consistently would alleviate the discomfort of both or either but if it's the adductor muscles that are causing you difficulty as described above you need to find additional stretching to work them out.  I agree that stairs and any knee lifting activity is illiapsoas related and if these are not symptomatic for you, you should look elsewhere.  Do you have access to a good,knowledgeable PT and or Deep Tissue massage therapist with above average anatomical assessment skills?  Do you practice a regular exercise maintenance routine?  Could it be impacting you negatively?
