What should i do? I have labrum degeneration age 32. Pros and cons of ceramic?

Small cam deformity, minor thinning of articular cartilage and intrasubstance degeneration of the labrum.

What does this mean in real terms. 

Also i have heard that ceramic on Ceramic resurfacing is the future what are the pros and cons?

Such a pity that nobody has replied to this. Still wondering the same myself... my husband is still on the fence, but options are limited.

It seems that most of his pain is caused by a labral tear, but OA is also present.

We are contemplating the CoC as well, but my husband (never been operated on) is scared as hell that something will go wrong and that he will not be able to run afterwards... We asked Dr De Smet about any runners in the Coc trial but he said so far there were none. However, I know of a female triathlete who did a sprint tri only 4 months after surgery, which is amazing.

I am trying to find running related experience after CoC, but it is hard to come by...

Sorry you are dealing with this at such a young age.  I did not really start running until I was 50 when I did 21 marathons (2 per year) and I feel blessed to be back to running at age 62 after Dr. Gross fixed me.

Only an experienced hip resurfacing surgeon can tell you if you need a hip replacement - hip resurfacing or THR. If you are having problems, I would suggest sending your x-rays to several of the top hip resurfacing surgeons by email. They give free email consultations.  Then you would know if you can have a hip resurfacing. Then you decide on the best surgeon you are comfortable with and they will decide what device would be best for you. Metal on Metal is the gold standard and when it is placed properly, they are almost no problems with them. If a less experienced surgeon places the angles wrong, then the bearing is misaligned and there is edge where causing high metal ions. So experience, experience and experience is the most important part of selecting a surgeon.  Only several are working with ceramic on ceramic now. None are approved in the US.  Always a bit of a gamble using new components. Obviously, someone has to have it to get the stats. But when the outcomes for metal on metal are so good, I personally would not worry.  The ceramic on ceramic were also partly developed so women and small men could have a hip resurfacing plus the metal ion worry.  The really experienced surgeons had no problems with doing a hip resurfacing for women or small men.  They still do it with devices other than the BHR.
So metal on metal definitely has the best track record in my opinion. I have a BHR which Dr. De Smet gave me in 2006. No problems then or now. I am a 77 year old woman. I hope to take it to the grave with me!

Find out if you can have a hip resurfacing first.  Then decide if you want to travel to Belgium to have ceramic on ceramic.  The H1 used by Professor Cobb is currently in trials only for folks in the UK.

I wholeheartedly agree with Pat. I am scheduled for CoC with Dr. De Smet in September. I sent my x-rays to a number of the surgeons on this site for evaluation. If you choose to go with one of them you are stacking the odds in your favor, but there is always some risk.

CoC prosthetics are still in trials, so based on their pedigree and properties I am hoping the ceramic materials are superior to MoM. Currently there is no hard data to support that claim.

The ReCerf (MatOrtho) CoC trials began in 2018. I would assume preliminary data might show up after 5 years or so. But what you really want is long term data. That is literally decades away.

For myself I am really basing my decision on the recommendation of my surgeon. That and my own (amateur) evaluation of the available literature. You (and your surgeon) will have to make your own decision on which way to go. Remember, you may not even qualify for CoC. And not every surgeon is using CoC prosthetics.

Putting off the surgery is one option. Unfortunately long term this will not be a viable alternative. And you do run a risk of limiting your options by putting if off too long.

Have you visited Dr. Pritchetts website?  It is comprehensive in its explanations of the options he offers.  There are several videos online as well where he discusses alternatives to MoM. including a non cemented cross poly prosthesis that has been very successful with his patients.  His surgical skill is among the very best in the world.  Not certain where you'd be travelling from or if it would even be a viable choice for you but worth considering.  Best of luck.
